You can save forms as custom templates and share them with team members to create several forms that have the same layout and functionality.To create a custom form template:

Create a directory to store your form templates. Save a form into that directory.

To add a custom form template:

1. Click Window > Preferences to open the Preferences window.
2. In the left column, expand Lotus Forms Designer.
3. Click New Form Templates.
4. Click New to open the Browse For Folder window.
5. Browse to the directory of forms you want to use as templates and click OK.You can use template forms saved to your own computer or stored on a shared network drive.
6. Click Apply and then OK. The forms in the directories you have selected will display in the New Lotus Forms Designer wizard.
7. If you want to change the order that the form templates are listed in the New Lotus Forms Designer wizard, click the file to highlight it, then click Up or Down to reposition the template.
8. If you want to remove a directory of templates, select it and click Remove.
9. If you want to use only the default templates, click Restore Defaults.

Click OK to close the Preference window.
